A number of users are presently reporting the following error when trying to leave module feedback (despite being enrolled on the correct course):

“Sorry you cannot take this unit evaluation form This could be because you are either not enrolled on the unit, have already taken the questionnaire or are not logged in”

Don’t panic.

The issue is presently being addressed and you will be notified as soon as it is once again possible to leave module feedback.

Apologies for the inconvenience.

UPDATE:

The issue has now been fixed and all users should be able to leave module feedback as normal.
